We understand that moving into Adderley Primary School, between year groups or on to another school can require additional planning for pupils with SEND. We work with the pupil, family, current staff, receiving staff and relevant professionals to identify what will support a successful transition. With appropriate consent, we share essential information about the pupil’s needs, strengths, outcomes, reasonable adjustments, successful strategies and current provision. Transition arrangements may include additional visits, photographs or visual information, meetings with key adults, familiarisation activities and a personalised transition plan. The SENCo liaises with the receiving school to support continuity and reduce disruption.
